"SNAP" - SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM (FORMERLY THE FOOD STAMP PROGRAM)

SNAP nutrition program is now easier than ever to apply for if you are 60 years of age or older. The application has been simplified and there is no longer an asset limit. You may obtain an application at the Foxboro Senior Center or online at www.mass.gov. Applications can be processed by mail.

COMMUNITY SOCIAL WORKER

The Foxborough Council on Aging and Human Services offers social services to help Foxborough residents of any age with benefits, counseling, application assistance and program referrals for services such as the Foxborough Food Pantry, Discretionary Fund or Department of Transitional Assistance. Pam McGuire, a licensed social worker, has an extensive social service background and is a wonderful advocate. She can be reached by calling the senior center at 508-543-1252 Monday - Friday between 8:00 a.m. and 3:30 p.m. Pam can arrange house calls to individuals who are unable to get to the senior center.

BLOOD PRESSURE CLINIC

The Blood Pressure Clinic is held twice a month at the senior center on the second and fourth Tuesday of the month, from 9:00 a.m. to 11:00 a.m. Transportation is available. This is a free clinic and is open to Foxboro residents of all ages by appointment only. Please call the office at 508-543-1234 to set up an appointment.

FRIENDLY VISITOR PROGRAM

This is a volunteer program offering socialization, errands, reading, etc… to individuals who have limited opportunities to leave their home and would benefit from a visit.

The Foxborough Council on Aging and Human Services has extensive, comprehensive resource files which are constantly being expanded and updated. HOME ENERGY ASSISTANCE PROGRAMS

The Foxborough Senior Center is the site for Self-Help fuel assistance applications for Foxborough residents. This is a federal fuel assistance program based on income eligibility. You may be eligible for assistance even if heat is included in your rent. You may also be referred to other energy assistance programs in the area.

LEGAL SERVICES AND ADVOCACY

Metrowest Legal Services (MWLS) is a nonprofit organization located in Framingham providing free legal services to people who live within our area and meet the income guidelines. They will provide free legal assistance in certain circumstances, help enroll people in benefit programs and provide representation during appeals. LOW VISION SUPPORT GROUP

Our Low Vision Support Group meets on the third Thursday of every month from 1:00 p.m. to 2:00 p.m. This is an adult group providing peer support and information for anyone with vision loss, or those who have concerns about their vision. Seniors from Foxboro, as well as surrounding towns, are welcome! Van transportation is available for Foxboro residents by calling the senior center at 508-543-1252.

MEDICARE PRESCRIPTION DRUG FINDER

Go to www.medicare.gov , click on "Compare Medicare Prescription Drug Plans" and you will be provided with a personalized chart comparing the Massachusetts Medicare drug plans. This interactive tool allows you to narrow your search for a Medicare prescription drug plan based on your personal preferences such as cost, coverage, and convenience. If you need assistance or don't have a computer at home, our SHINE counselor, Judy Murphy, will make a time to meet with you at the senior center in the Computer Lab to input your information. Call 508-543-1252 to arrange an appointment.

"PRESCRIPTION ADVANTAGE"

Do you have a prescription plan, but think you are still paying too much for your prescriptions? You may be paying more than you need to. You may be eligible for the Commonwealth's "Prescription Advantage" plan, which is currently enrolling new members. If your income is under $52,000 (individual) or $70,000 (couple) you may be able to reduce some of your prescription expenses. There is no 'open enrollment' limitation for "Prescription Advantage." You can apply at any time. Call Noreen Sherys at the senior center, 508-543-1252, to find out more or to apply.

GATRA DIAL-A-RIDE

GATRA Dial-A-Ride is door-to-door service within a 15-mile radius of Foxboro. All vehicles are handicap accessible. Anyone over 60 is eligible. Individuals under 60 with a disability are also eligible. There is a requested donation of $1.25 - $5.00 per ride, depending on your destination. These donations enable this valuable program to continue. Service will be provided Monday through Friday between 8:00 a.m. and 4:00 p.m., but may be subject to availability. LONG DISTANCE MEDICAL TRANSPORTATION OFFERED THROUGH GATRA

GATRA is now contracting with Foxfield Taxi for long distance medical transportation. Foxfield's "Miles for Health" is a new long distance medical transportation program for persons 60 years of age or older and for those who are under 60 years of age but have a disability. Transportation to appointments in Boston will be available on Mondays through Fridays between the hours of 9:00 a.m. and 1:00 p.m. All other long distance medical trips such as Burlington, Framingham, Newton-Wellesley, Worcester, Rhode Island, etc. will be scheduled for Wednesdays also for appointments made between the hours of 9:00 a.m. and 1:00 p.m. Return trips will be scheduled for 2:00 p.m. or 4:00 p.m. Advanced notice of 48 hours is required. Reservations will be taken from 8:00 a.m. until 4:00 p.m. Monday through Friday by calling 800-585-8294. Please note that this is a separate phone # from Dial-A-Ride. A requested donation of $10 for a round trip may be made through the Foxboro Senior Center or directly to GATRA.

OPERATION REASSURANCE

The Operation Reassurance program is a free service for Foxboro residents. Participants check in each day Monday through Friday via telephone to ensure that they are O.K. This program is wonderful for people who live alone, those confined to their home due to illness, or anyone who wants the comfort of knowing someone will check on them once a day. Call the senior center at 508-543-1252 for more information.
